Purchase the plan. Your official membership kit will arrive in the mail within 7 to 10 business days.

Step 2

Locate a participating provider here or by calling (800) 441-0380 Ext. 2909, any time between 8:30 a.m. and 5:30 p.m., CT and we can help find a provider in the network that accepts your plan.

Step 3

At the time of service, show your member ID card and receive a discount off the regular-priced fee. It's that easy.
